| General Land Planning Feedback Form | | Please use this form to communicate your opinion to the PDPOA Board of Directors on any or all of the land planners proposals for Palmetto Dunes.

| Mailbox and House Number Yard Sign | | Please submit this form for all mailbox repair or replacement requests or to purchase a house number yard sign. Mail check or money order to:

| New Property Owner Form | | All new property owners are required to complete the following form. If the property is listed under a Partnership, Trust, Business, or has multiple owners, each owner must fill out a separate form.

| Request a Pass | | This form allows you to request a pass or passes for access to Palmetto Dunes. Passes will be available at the Security Pass Office within 24 hours of this request. You will receive a confirmation copy of your request from the website.

| Security House Check Form | | By request for those owners away for a period of time
▪ Initial check of the exterior will be performed upon departure
▪ Perimeter checks performed a minimum of three times weekly
▪ Drive by checks will be performed daily
▪ Security will report any infractions found
▪ To obtain this service, simply submit this on-line form
▪ If you prefer, you may pick up a form from the Security Office or any Guard Gate, fill it out and drop it off at the Security Office.

| Security Lockbox Request | | The Palmetto Dunes Security Department will place a lockbox at your home for a fee of $85.00. You supply Security with a key to your home, as well as your security code, which is placed in the lockbox. Our Patrol Officers have the means to open the lockbox with a special key.
